Artist's Check List

Be certain all of the following are enclosed:

• Completed application/entry form(s), signed, including your Wisconsin Seller’s Tax Number or Social Security Number and Federal Identification Number.
• Completed Competition Entry Form.
• $15.00 ($10.00 for exhibitors) separate check jury fee per entry, nonrefundable.
• $50.00 separate check for exhibitor fee, returned if applicant is not selected to exhibit. Check is deposited after selection jury has made their decision.
• Three (3) slides or photographs of current work, no glass mounts.
• Addressed, stamped return envelope for entries and notification.
